The following terms and conditions (the “Submission Terms”) govern the submission of your registration form to a course or program provided by NSI (the “Application”) through the NSI Sites (as defined in our Terms of Use). These Submission Terms constitute a binding agreement between you and NSI, and are in addition to our Terms of Use. The Terms of Use are hereby incorporated into these Submission Terms by reference. To the extent that the Submission Terms are inconsistent with the Terms of Use, these Submissions Terms will prevail. BY SUBMITTING THIS REGISTRATION FORM FOR THE ELEVATE LEARNING MODULES (THE “PROGRAM”), YOU (THE APPLICANT) ACKNOWLEDGES THAT YOU AGREE TO PARTICIPATE IN THE PROGRAM. YOU HEREBY CERTIFY THAT TO THE BEST OF YOUR KNOWLEDGE ALL INFORMATION PROVIDED IN THIS REGISTRATION FORM IS CORRECT AND COMPLETE.
